If a student intends to play a Fall sport, their completed physical form must be received by the athletic office PRIOR to the first practice. Please consult coaches for the date of the first practice. If a physical is still needed, contact your family physician. Sports physicals are also offered at Northeast Internal Medicine in LaGrange for $25. Please call 260-463-2133 for an appointment.
